Privacy Notice
As part of the service I provide I require to hold some of your personal information.
I, Dr Shona Fraser, will ensure that any information you provide will be kept confidential (under the Data Protection Act 1998 and the General Data protection Regulation 2018).
I will not disclose this information to other individual or external agencies unless I am legally obliged to do so as detailed below:
- if I have concerns about the safety of a child or a vulnerable adult.
-if something is said that suggests you have benefitted from the proceeds of crime (under Proceeds of Crime Act 2001)
-if there is violence, or threat of violence before, during or after a session where I, the client, or another party have felt it necessary to call the police or where I am a witness to an incident which results in criminal proceedings against a client
- if a disclosure is made relating to a serious crime, or if I have serious concerns about your safety as I may have a duty to report this to the authorities
I have a duty to keep my records for 7 years – after this time your data will be securely disposed of.
-the demographic data (i.e. name, age, family circumstances, occupation etc) are kept separate from the session records. Both are kept under lock and key. I never carry both copies together. This is to ensure that the contents of a session cannot easily be identified with a specific individual person.
-telephone numbers are stored on my mobile phone under initials only so cannot be identified
-email addresses are kept in my diary which is under lock and key. All email correspondence is deleted once dealt with.
I do not record sessions without the explicit authorisation of the client.
You have the right to access the personal data I hold about you. If you wish to raise a concern about the way I hold your data then please contact me. If you are then not satisfied with my response, or believe I am not holding your personal data in accordance with the law, you are free to make a complaint to the Information Commissions Office (ICO)
